Scholarship Application

Mentorship Program Agreement I agree to participate fully in the mentorship component of this program. I understand that mentors are available to me to further my career and educational achievement. I understand that I can request a mentor change at anytime and will voice any concerns in a timely manner to appropriate leadership.

Collegiate Academic Standards Agreement I agree to maintain a grade point average of 3.2 and adhere to all university policies and procedures. I understand that I am not to unenroll or transfer between institutions without first notifying my mentor or appropriate program leadership.

Image Release Agreement I agree to the release of my name and image on appropriate Foundation web and social media sites if selected to receive scholarship funds.

Original Work Agreement I affirm that the below personal statements are my original work and that I am not submitting work completed by another individual and/or generative AI. I understand that failure to uphold the standards of the Opal and Clinton Barrett Foundation for Rising Communities may jeopardize my scholarship disbursement.
